Contractor Requirements

Vendor Requirements

 

Please review the following to confirm if you meet all criteria to serve as a vendor/ provide contracted services to Sunrise of Philadelphia:

• Fully executed Scope of Work.

• Provide current certificate of insurance including worker's compensation and general liability insurance.

• W-9

  • If a Social Security Number is provided on the W-9 you will also need to complete an I-9 form

• I-9 Form

  • only needed if a Social Security number is provided on the W-9

• Provide invoice for service(s) 

• Receive payment within 30 days of Sunrise receiving final invoice once service(s) is complete.

  • For longer engagements, Vendor may submit progressive invoices on a schedule agreed upon in the Scope of Work for services rendered (i.g., may submit Progress Billing).

 

If you will be performing services at any of our locations you must also provide:

• Provide following clearances for all instructors

  • PA State Police Clearance 

  • Child Abuse Clearance

  • FBI Clearance (fingerprints): These can take several weeks to be returned

  • NSOR: This will take several weeks
